Understanding Targeted Case Management Services

Targeted Case Management (TCM) has been an avenue of interest for many districts and here are tips to help you consider the options available for maximizing your reimbursement.

Targeted case management services assist eligible children and eligible family members to access needed medical, social, educational and other services when TCM is covered by the student’s IEP or IFSP. These services are reimbursable when provided by qualified staff deemed as eligible providers.

Components of TCM include determining student needs, developing a plan of care and coordinating services, including assessing services outside the school system.

The first steps to claiming targeted case management services is to complete a TCM Labor Survey to qualify your district for reimbursement and to determine your fee schedule. This survey is done on an annual basis and is due October 10th of each school year. DHCS is allowing your district to submit this survey this school year and once approved the district may submit services provided retroactively for prior services up to 12 months from the date of service.

The key to successful claiming is being well informed, following recommendations and guidelines in your current State plan, and documenting all services provided.
