Connecticut Re-Enrollment Applications are due for many school districts participating in School-Based Medicaid

CT Renrollment for school based medicaid

Our Connecticut school districts have been notified by the Department of Social Services that your enrollment applications are available and must be completed and submitted to Hewlett Packard (HP) your fiscal billing agent.

In order for school districts to continue participating in the School Based Child Health (SBCH) program they must complete their re-enrollment application. Many of our clients have reached out to CompuClaim for assistance in this application process and we are providing you with a simple to follow guide to complete your application.

For the past year through a collaborative effort between the Department of Social Services -SBCH program, DSS Medical Operations, DSS Office of Legal Counsel, DSS Office of Quality Assurance, and Hewlett Packard Provider Enrollment, a newly revised application has been created.

All districts previously enrolled in the program must complete this application within 30 days from the time the application request was sent to you. On October 9, 2013 DSS sent this notification through email to all districts whose enrollment has expired.

The application can be found on the DSS website:
